- A statement from you, under penalty of perjury, that you have a good faith belief that the material was removed or disabled as a result of a mistake or mis-identification of the material to be removed or disabled;
- a detailed explanation with justification why you believe the material should not have been removed or disabled; and your physical or electronic signature.
